Suicidal ideation and behaviors are reported in immune-mediated skin diseases, including atopic dermatitis, psoriasis, and hidradenitis suppurativa. An overview of systematic reviews with meta-analysis was conducted with the aim to summarize the epidemiology of suicide in immune-mediated skin diseases. A systematic search was conducted in CINAHL, EMBASE, PsycINFO, and PubMed. Reviews focusing on immune-mediated skin diseases, assessing suicidal ideation, attempts, behaviors, or suicide deaths, were included. Methodological quality was assessed using AMSTAR 2, and overlap was calculated. Descriptive analyses and world maps explored regional disparities. Twelve reviews were included, addressing atopic dermatitis, psoriasis, and hidradenitis suppurativa. Pooled results were reported for suicide outcomes. In atopic dermatitis, the risk of suicide attempts was OR = 1.36, suicidal ideation ranged from OR = 4.32 to 1.44, and suicidal behavior was OR = 1.53. For hidradenitis suppurativa, two reviews reported ORs for suicide deaths of OR = 2.86 to 2.24. In psoriasis, pooled estimates were RR = 1.13 and HR = 1.33 for suicide deaths, RR = 1.25 and HR = 1.22 for suicide attempts, OR = 2.05 to 1.97 and HR = 1.74 for suicidal ideation, and OR = 1.42 to 1.26 and HR = 1.08 for suicidal behavior; however, several of these estimates crossed the null value. High overlap existed for most outcomes and diseases except suicidal behaviors in psoriasis. Geographical disparities were noted, with no primary studies from South America or Africa. The available evidence suggests increased risk for some suicide-related outcomes in atopic dermatitis, psoriasis, and hidradenitis suppurativa. However, several pooled estimates, particularly suicide deaths and attempts in psoriasis, crossed the null value and should be interpreted with caution. Different effect measures (OR, RR, and HR), high heterogeneity, and overlap between reviews limit direct comparison and generalisability of the pooled estimates.
